1. Complete your new client forms– so that your consultation is seamless, personalized, and backed by our preparation for the best outcome.

2. Prepay your consultation – this secures your dedicated time with Dr. Pessoa and is applied in full to your smile enhancement treatment. 

Once these steps are finished, your consultation will be confirmed and your reserved time secured.

Because our consultations are reserved exclusively, if the steps are not completed within 48 hours from the submission of the form below, the time you selected will be opened to others waiting for a consultation with Dr. Pessoa.
